14 pioneering initiatives for co-ordinated care

In England and Scotland, fourteen initiatives are transforming the way health and care is being delivered to patients by pioneering new ways of delivering coordinated care. 100 local areas initially applied for pioneer status and from the 28 shortlisted, 14 have been selected to focus their efforts in different ways and test a range of models in moving towards more coordinated care. While the pioneers are at the front of this curve and are expected to produce action learning for everyone, all areas are being encouraged to develop coordinated care plans in order to access money from the Treasury’s £3.8bn integration fund.
